HMTS Team Entry Form

All teams fishing the Hawaii Marlin Tournament Series must submit a completed Team Entry Form in advance of the first day of fishing. You can download the form and bring the printed copy with you to registration, or you may complete the form below and slick SUBMIT at the bottom.

Team Entry Forms are required for ALL entries.

Forms can be amended as needed. If you need to update previously submitted Team Entry Form information, please do NOT submit another form, but rather email [email protected] with your name, boat name, and the changes you would like to make.

Team Base Entry Fees for all tournaments covers up to four Anglers and two Crew. Any additional team members can be added on for a fee of $50 each. If you have paid for extra team members, please email their information to [email protected].
